登録後に表示されるURLから登録内容の修正が可能です．

- 参加登録サイトの入力項目はどれも必須ではありませんが，今後のお知らせの際に連絡先を使用させていただきますので，お名前と連絡可能なメールアドレスのご入力をお願いいたします．

今回個人情報を収集する目的は，林茂生研究室の同窓会名簿作成です．作成した名簿は，林茂生先生の還暦パーティー，還暦関連シンポジウムなど林茂生先生個人に関する行事にのみ使用します．個人情報保護の保護に関する情報のガイドライン（通則編）に準拠し，いただいた情報は通信経路の暗号化及び保管時の暗号化を施し，データにアクセス可能な人を管理します．
- The URL for participation registration sites will be sent by e-mail. Please fill in the registration form, congratulatory message, contact information, etc.
- Even if you are not participating, we are glad to take your congratulatory message to Dr. Hayashi. If you have “Photos related to Hayashi’s laboratory”, please provide them (Photo registration site). We are thinking of using them at the celebrating party.
- After the save(「送信」) button click, the personalized URL will be generated. This URL is important for review/modify registration or deposite photos, please make sure the URL is actually saved for later use.
The purpose of collecting personal information is to create a reunion directory of Shigeo Hayashi laboratory. The created directory is used only for the Shigeo Hayashi’s 60th birthday party, the 60th anniversary related symposiums, and other events related to Shigeo Hayashi. In accordance with the guidelines (general rules) on the protection of personal information protection, we will encrypt the communication path and the stored information, and restrict the person who can access the information.
